If you have already booked your first appointment with us via telephone or online booking, you will have received an email or a text from us, forwarding you to this page.
We would like you to enclose all the information YOU think is vital for us to know before you walk through our doors. You don’t have to fill out every slot – we can do that as we go along. It is entirely up to you. Maybe you have some specific requests, demands or perhaps you are a bit anxious going to a new dentist?

PRACTICAL INFORMATION:
- Yellow Health Card (CPR) must be handed in
- VISA Dankort or cash (MobilePay)
- You must turn up 15 minutes before your appointment, if you haven’t previously used the “New Patient” form, otherwise 5 minutes before the appointment
- We will help you obtain your previous dental records,if that has not already been done via the “New Patient” form
